29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Jobsite: 139 Modena Rd, Coatesville, PA, Nickel Clad 1 Building, #009 Giddings Lewis Milling Machine, steel oscillating belt, mechanical roller - Employees performing housekeeping activities under the mechanical roller were exposed to amputation hazards as the two mesh "ways" guards covering the mechanical roller were not constructed as to prevent the employees from having any part of their bodies in the danger zone during the operating cycle. Noted on or about 10/24/19.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i). a) Jobsite: 139 Modena Rd, Coatesville, PA, Nickel Clad 1 Building, Column #20, 600 VAC Westinghouse Circuit Breaker Box -Employees working within two feet of an open 600 VAC circuit breaker box were exposed to electrical shock, burn and electrocution hazards as the contacts of the live wiring inside the circuit breaker box was not guarded against accidental contact by the use of approved enclosures. The cover on the enclosure was damaged and would no longer stay fully closed. Noted on or about 10/24/19.
