29 CFR 1910.305(g)(2)(ii): Flexible cords were not used only in continuous lengths without splice or tap: JOB SITE: 300A Industrial Drive, Troy, MO 63379, on or about 10/24/2019, in the production area, an orange flexible cord connected to a floor fan, was not used in continuous length without splice or tap. The flexible cord's insulation was damaged exposing wiring and was wrapped in black electrical tape. The workers were exposed to a potential electrical hazard.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting an in-patient hospitalization, amputation or the loss of an eye: JOB SITE: 300A Industrial Drive, Troy, MO 63379, on or about 10/24/2019, The employer fail to report within 24-hours a work-related incident resulting in an amputation injury. The maintenance technician was working on a Print Head moving cart when his right middle and ring fingers tips were amputated. The incident occurred on October 16, 2019 and was reported to OSHA on October 22, 2019.
