29 CFR 1926.200(a): Required signs and symbols were not visible when work was being performed: (a) On or about October 23, 2019 in Building B1, employees moved appliances into spaces on the 4th floor including, but not limited to apartment 413. Fall hazard signage had been temporarily removed in order to properly install a door frame and door and had not been put back in place to warn employees against a fall hazard.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) On or about October 23, 2019 in Building B1, workers delivering appliances to apartment units near a stairwell, where the stairwell door was left temporarily unbarricaded, were exposed to a fall hazard greater than six feet to the next lower level. The door was temporarily left unbarricaded after work was done to the doorframe and could not be locked.
