29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. 152 Bethany Rd, Monson, MA: On October 24, 2019 and at times prior, the location of the pneumatic source of energy listed on the TMI machine #2 energy control procedure was not accurate, did not specify types of isolating/lockout devices, and was not able to be located by an authorized employee.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: 152 Bethany Rd, Monson, MA: As of October 24, 2019, the employer had not performed an annual periodic inspection that ensured each procedure was inspected for inaccuracies during an actual energy control task. The employer also did not ensure that the inspector reviewed each authorized employee's responsibilities under each energy control procedure.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: 152 Bethany Rd, Monson, MA: On October 23, 2019 and at times prior, employees who operated the TMI machine #2 in the reverse mode were exposed to the unguarded nip point between the fabric and the rotating roll receiving the fabric. On October 23, 2019, an employee operating that machine received hand and arm injuries in that unguarded nip point.
