Powerline Rd Pompano Beach FL 33069 two (2) inch line saw area, an employee operating a cutting saw was exposed to a noise level of 91.3 dBA, which exceeds the 85.0 dBA action level during the 8-hr representative sampling period.     b) On or about October 30, 2019 at 1543 N. Powerline Rd Pompano Beach FL 33069 three (3) inch line saw area, an employee operating a cutting saw was exposed to a noise level of 90.3 dBA, which exceeds the 85.0 dBA action level during the 8-hr representative sampling period.     c)  On or about October 30, 2019 at 1543 N. Powerline Rd Pompano Beach FL 33069 doors department, an employee operating a router, saw and drill press was exposed to a noise level of 89.5 dBA, which exceeds the 85.0 dBA during the 8-hr representative sampling period.     d)  On or about October 30, 2019 at 1543 N. Powerline Rd Pompano Beach FL 33069 three (3) inch glazing area, an employee operating a cutting saw was exposed to a noise level of 87.0 dBA, which exceeds the 85.0 dBA during the 8-hr representative sampling period.

29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:  Instances:  a) On or about October 30, 2019 at 1543 N. Powerline Rd Pompano Beach FL 33069 two (2) inch line saw area, an employee operating a cutting saw was exposed to a noise level of 91.3 dBA, which exceeds the 85.0 dBA action level during the 8-hr representative sampling period. The employer failed to provide the employee with a baseline audiogram within six months of an employee's first exposure above the action level and subsequent annual audiograms.  b) On or about October 30, 2019 at 1543 N. Powerline Rd Pompano Beach FL 33069 three (3) inch line saw area, an employee operating a cutting saw was exposed to a noise level of 90.3 dBA, which exceeds the 85.0 dBA action level during the 8-hr representative sampling period. The employer failed to provide the employee with a baseline audiogram within six months of an employee's first exposure above the action level and subsequent annual audiograms.  c)  On or about October 30, 2019 at 1543 N. On or about October 24, 2019 at 1543 N. Powerline Rd Pompano Beach FL 33069 welding area,  a flammable chemical such as Acryl-R acrylic self-leveling sealant was dispensed in the immediate area next to welding activities, exposing employees to a fire hazard.
