29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) Construction site, Building C, 5th Floor - On or about October 21, 2019, an employee used an in-series combination of an approximately 4 foot long anchor strap, an 18 inch long D-ring extension, and 6 foot long shock absorbing lanyard rigged together which allowed for a free fall greater than 6 feet before beginning to arrest the fall.
