29 CFR 1960.8(a): The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm: Naval Air Weapons Station, China Lake: On or about July 7, 2019 and thereafter, employees were exposed to heat-related illness while performing high voltage restoration on electrical equipment without being acclimatized prior to working in extreme temperatures.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. Naval Air Weapons Station, China Lake: On or about July 13, 2019, employees were performing high voltage electrical work in temperatures up to 105 degrees F. An employee was hospitalized due to heat illness-related symptoms. A late notification to OSHA was made on October 17, 2019.
