29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by the use of a guardrail system, safety net system, personal fall arrest system, warning line system, or a safety monitoring system.  a) 588 Hazle Street, Wilkes-Barre:  Employees were not protected from falling by use of a guardrail system, safety net system, personal fall arrest system, warning line system or a safety monitoring system while performing roofing construction activities from a low slope (2:12 pitch) roof, exposing employees to a fall hazard of more than 35 feet to the ground level, observed on or about October 25, 2019.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:  a) 588 Hazle Street, Wilkes-Barre, PA: An aluminum extension ladder (Mfr unknown) used by employees to gain access to the roof was placed in the roadway and was not secured to prevent accidental displacement nor was a barricade used to keep the activities or traffic away from the ladder, exposing employees to fall or struck-by hazard, observed on or about October 25, 2019.
