29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: (a) Centre Pointe Shopping Plaza, Rear side of dance studio building: On or about 10/25/2019, and at times prior, the employer exposed employees to overhead struck-by hazards, in that hard hats were not provided to ground workers who were located directly below the aerial lift as employees were installing Dens Glass Sheathing.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee working from an aerial lift: (a) Centre Pointe Shopping Plaza, Rear side of dance studio building: On or about 10/25/2019, and at times prior, the employer exposed employees to ejection/fall hazards of 26-feet, 3-inches to the lower level, in that a personal fall arrest system was not provided while working from the JLG 460SJ Aerial Lift basket installing Dens Glass Sheathing.
