29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  Worksite located at 3531 Gillham Rd, KC MO -- Employee was pressure washing a flat roof of a commercial building with no fall protection, guardrails or safety net systems.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Worksite located at 3531 Gillham Rd, KC MO -- Employee and Employer were using a portable extension aluminum extension ladder to gain access to the roof of the commercial building. The ladder was not extended 3 feet above the landing surface, in this case, the roof. Employee and employer were both exposed to fall hazards from this.

29 CFR 1926.1053(b)(6): Ladders were used on surfaces which were not stable and level:  At worksite 3531 Gillham Rd, KC MO -- Employee and employer were using an aluminum extension ladder that had the feet of the ladder on the flatbed of the pick up truck versus on the ground. The truck was parked and in the off position. However, the ladder was not made stable due to the truck could have rolled.

29 CFR 1926.1053(b)(12): Ladders used where the employee or the ladder could contact exposed energized electrical equipment did not have nonconductive side rails:  Worksite located at 3531 Gillham Rd, KC MO -- Employee and employer used an aluminum extension ladder very close to energized high voltage electrical wires. The aluminum extension ladder did not have nonconductive side rails.
