29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  Employee(s) engaged in maintenance activities was exposed to struck-by and caught-between hazards.  The employer had not developed or implemented a written energy control program containing all the elements required in 1910.147(c).

The employer did not perform powered industrial truck operator training.  The employee driving the John Deere 324K forklift, moving materials throughout the production areas, had not been trained.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Employees engaged in stone split operations were exposed to amputation hazards.  The stone HYDRASPLIT QS-1000 Splitter machines did not have adequate point of operation guards. The employee's hands were holding the stone during the spliting process.  1) West Splitter machine was missing point of operation guarding. Employees were exposed to hazards such as but not limited to struck by and amputation hazard.  2) East Splitter machine was missing point of operation guarding to protect employees from hazards such as but not limited to struck by and amputation hazards.

29 CFR 1910.304(g)(5): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective.  Employees were exposed to electrical hazards.  At the West Hydra Splitter machine, the electrical cord of the PROSERIES battery charger was not provided with a continuous path to ground.  The ground pin was not properly plugged into the ground of the electrical socket.
