29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, or personal fall arrest system:    a) Jobsite: On October 16, 2019 an employee performing roofing activities on a flat roof did not properly use the personal fall restraint system that was in place.  The harness the employee was wearing was not attached to the lanyard as part of a complete personal fall arrest/restraint system. This exposed the employee to a 12 foot fall hazard when working and getting on/off the roof at the ladder.       Note) Corrected during the inspection.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:    a) Jobsite: On October 16, 2019 and at times prior thereto employees performed roofing activities on a flat roof using  fall restraint systems (rope grabs).  The rope grab systems were adjusted such that they would not have prevented the employees from reaching the edge of the flat roof and potentially falling to the ground below.      Note) Corrected during inspection.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:    a) Jobsite: On October 16, 2019 and at times prior thereto a written certification had not been completed verifying employees had been trained in proper use and operation of the fall protection system in use and hazards related to falls from elevations.
