29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee was using appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    Facility: The employer did not ensure through not only providing appropriate training, but also visual observation that the employees required to work with and handle hazardous chemicals, such as (but not limited to) DeClog (contained 60-100 % sulfuric acid), Kling (contained hydrogen chloride >5 - <1 %), E-100 (contained sodium hydroxide 40-70 % & sodium nitrate 15-40 %), and Miracoil ll (contained sodium hydroxide 10-20 %) were not only provided with but also wearing appropriate eye and/or face protective equipment.    On or about October 18, 2019, sulfuric acid splashed onto an employee who was using DeClog (contained 60 to 100 % sulfuric acid) to clean a toilet, and the employee sustained 2nd/3rd degree burns on hands, one of arms, and one of eyes.
