29 CFR 1910.305(g)(2)(ii): Electrical cable was repaired by splicing, which did not retain the insulation, outer sheath properties, and usage characteristics of the cord that was spliced:Note: Hard service flexible cords No. 12 or larger may be spliced if the splice retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced. On or about October 21, 2019, there was an electrical cable which was repaired by splicing. The splice did not have an outer sheathing in place matching the properties and usage characteristics of the cord being spliced. . This condition exposed the employees to potential electrical hazard.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: On or about October 21, 2019, there were at least 2 instances where the outer insulation was pulled back at the connection, exposing the inner conductors.
