Workers were installing siding and other trim pieces using a pneumatic nail gun powered at 150 psi without the use of eye protection. the following condition exposed employees to struck-by hazards to the eyes.

Workers were on a pump jack scaffold at heights between 10-18 feet installing siding and other trim pieces. The pump jack scaffold did not have guardrails nor was any other method of fall protection utilized. The following condition exposed employees to falls from up to 18 feet.

A pump jack scaffold was being utilized with an extension ladder being used as a work platform for tools and equipment. The area beneath the scaffold was not barricaded nor was a toeboard or other means to prevent tools/equipment falling and striking employees utilized. The following condition exposed employees to struck-by hazards.

A pump jack scaffold was being utilized at heights between 10-18 feet for siding installation. The bottom section of both poles supporting the scaffold were not secured or braced in any way.
