On or about 10/28/2019, the employer did not instruct each employee in the recognition and avoidance of unsafe conditions related to the dismantling procedures when removing a Strickland grapple attachment on a CAT 349E L Hydraulic Excavator. The attachment was not properly chocked and blocked to insure that there was no stored energy in the machine prior to starting the dismantling process.
