29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Work located on the 2nd and 3rd floor of building. On or about October 30, 2019, employees were not protected from falls while bringing electrical materials into the building with the assistance of a mobile crane. Employees were exposed to fall hazard of more than six feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about October 30, 2019, and at times prior thereto, employees working near the 2nd and 3rd floor exterior door of the building were not provided a training program to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards. Employee were exposed to a fall hazard of more than six feet.
