29 CFR 1926.451(d)(3)(viii): Outrigger beams were not placed perpendicular to its bearing support (usually the face of the building or structure). However, where the employer can demonstrate that it is not possible to place an outrigger beam perpendicular to the face of the building or structure because of obstructions that cannot be moved, the outrigger beam may be placed at some other angle, provided opposing angle tiebacks are used. Location: Location: 515 Elizabeth Ave., Newark, NJ.  a) An employee used a two point suspended scaffold but did not tie off the lanyard to the life line.  The employee was exposed to a potential fall hazard of approximately 40 ft. to the floor below; on or about 10/28/19.  b) An employee walked/worked from a bridge scaffold that was missing the parapet wall at the access point. The employee was exposed to a potential fall hazard of approximately 15 ft. to the floor below; on or about 10/28/19.
