29 CFR 1910.22(d)(2): Hazardous conditions were not immediately repaired or properly guarded before an employee used the walking-working surface. On or about October 22, 2019, Cactus Rig 139, employees were exposed to trip/fall hazard due to an unstable metal grate not being flush to the surface. While stepping on the metal grate, it stuck out about 2 1/2 inches from the even surface.
