29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial trucks were performed by the employer without the manufacturer's prior written approval: a) Warehouse - On or about August 30, 2019, the employer failed to ensure the Hyster forklift, model #S80XM, serial #F004V03020C, that used the blue boom attachment was provided with the manufacturer's prior written approval and marked to identify the attachments and show the approximate weight of the truck and the attachment combination at the maximum elevation with the load laterally centered. No abatement certification or documentation is required for this item. b) Warehouse - On or about February 3, 2020, the employer failed to ensure the Raymond forklift, model #750-DR32TT, serial #750-14-BB41727, that used the light pole attachment was provided with the manufacturer's prior written approval and marked to identify the attachments and show the approximate weight of the truck and the attachment combination at the maximum elevation with the load laterally centered.
