29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial trucks were performed by the employer without the manufacturer's prior written approval:    a) Legacy Farmers Cooperative located at 22930 Defiance Pike in Custer, Ohio; On or about October 30, 2019 the employer did not obtain written approval from the manufacturer prior to utilizing a man basket attachment on a Hyster Lift Truck, Model H65XM. Employees being lifted in the man basket to gain access to entry points on grain storage structures were exposed to fall hazards in that the man basket attachment which can affect the capacity and safe operation of the forklift was not approved by the manufacturer.

Employees were exposed to engulfment hazards in that the infeed grain conveyor gate was locked out prior to entering grain silos for cleaning.
