29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6): (a) Jobsite, 505 N. Euclid Avenue, Pittsburgh, PA: On or about October 30, 2019 the pump jack scaffold being used by employees to work on the trim around the windows had been erected within 10 feet of the 4kV power lines exposing employees to potential electrical shock and burn hazards.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: (a) Jobsite, 505 N. Euclid Avenue, Pittsburgh, PA: On or about October 30, 2019 no fall protection systems were in place on the pump jack scaffold. Two employees were working on the scaffold and the fall hazard was approximately 24 feet to the ground below.

Euclid Avenue, Pittsburgh, PA: On or about October 30, 2019 the employer did not train employees who worked on the pump jack scaffold on hazards associated with such work that included potential electrical hazards and fall hazards.
