29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound toprail capacity):  a) Scaffolding System A (North Side of Structure): On or about October 28, 2019, the employer did not ensure guardrails were provided and used on all open sides which exposed employees to falls up to approximately fifteen (15) feet to the ground below.  b) Scaffolding System B (South Side of Structure): On or about October 28, 2019, the employer did not ensure guardrails were provided and used on all open sides which exposed employees to falls up to approximately fifteen (15) feet to the ground below.   The E.B. Masonry, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1)(vi), which was contained in OSHA inspection number 1108593, citation number 1, item number 1 and was affirmed as a final order on December 30, 2015, with respect to a workplace located at 6377 Old Shell Road Mobile, Alabama 36608 USA.
