29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about October 28, 2019, and at times prior thereto, while removing and installing guttering the employer failed to instruct employees to recognize and avoid hazards including but not limited to the following instances: a) Employees brought conductive material within the minimum approach distance and contacted live overhead electric lines exposing employees to electric shock hazards. b) Employees operated aerial lifts without adequate training to avoid hazards exposing employees to electric shock and fall hazards.

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: On or about October 28, 2019, and at times prior to, the employer did not ensure that employees removing and installing guttering were protected from contact with energized electrical circuits by de-energizing and grounding or guarding the circuits by insulations or other means. Employees were exposed to electric shock hazards.
