29 CFR 1910.28(b)(12)(i): Each employee on a scaffold was not protected from falling in accordance 29 CFR part 1926, subpart L: On or about October 30, 2019 and at times prior thereto, employees using a platform attached to a Powered Industrial Truck (PIT) as a scaffold did not have guardrail systems installed that were between 36 inches and 45 inches above the platform surface when personal fall arrest systems are required, exposing employee(s) to the hazards of falling from heights greater than 4 feet.

On or about October 30, 2019 and at times prior thereto, ATX Fulfillment, did not record a work-related injury were an employee assigned to take apart warehouse pallet racking fell from a height greater than 4 feet without the use of fall protection, resulting in the employee needing medical treatment and days away from work.
