29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: a) Hi-Grade Welding and Manufacturing, LLC, Schaumburg, IL- On October 30, 2019, employees voluntarily using respirators such as the 3M elastomeric half mask respirators, the Powered Air Purifying Respirator (PAPR) , the GVS Elipse SPR 321 P100 elastomeric half mask respirators and the 3M Aura 9211 N95-rated disposable respirators with filtering facepiece were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user: a) Hi-Grade Welding and Manufacturing, LLC, Schaumburg, IL- On October 30, 2019, the employer had not established and implemented those elements of the program necessary to ensure that any employee voluntarily using a respirator was medically able to use that respirator, and had knowledge on its appropriate use, maintenance, cleaning and storage, such as the welders using 3M elastomeric half mask respirator(s), 3M elastomeric half mask respirators, the Powered Air Purifying Respirator (PAPR) , the GVS Elipse SPR 321 P100 elastomeric half mask respirators. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
