29 CFR 1910.1200(j)(2)(ii): The employer did not update their written hazard communication program required by paragraph (h)(1) by June 1, 2016 to reflect changes in labeling and safety data sheets: Employees engaged in pellet machining activities throughout the shop were exposed to chemical hazards in that the written hazard communication program developed on December 2010, Revision 2 had not been updated in accordance with the revised hazard communication standard. Employees using hazardous chemicals, including but not limited to: Irganox B 215 FF, Sachtolith HD-S, Marlex 1017 Polyethylene, Linear Low Density Polyethylene, Tinuvin 622 SF, and Chimassorb 944 FDL. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: Employees engaged in allocating material for production and warehouse activities in the warehouse area voluntarily using 3M N95 respirators were exposed to respiratory hazards in that the information contained in Appendix D was not provided. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
