Location: Jobsite public street in the vicinity of 2451 Washington Street Boston MA. On or about October 31, 2019, employees were exposed to cave-in hazards while working in a 5 foot-4 inch deep trench that had straight cut walls, without cave-in protection.

On or about October 31, 2019, employees were exposed to hazards associated with trench work such as cave-ins and caught-between while working were a competent person inspection had not been preformed.
