29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice: (a) On or about October 30, 2019, employees were exposed to a tripping and fall hazard while performing work on the train shed roof. Mounting bolts (Twelve (12) separate locations) used to mount equipment (ventilation) inside of building were raised approximately two (2) inches up from the roof.

29 CFR 1910.30(a)(1): The employer did not provide training for each employee exposed to fall hazards and who used personal fall protection systems or is required to be trained as specified elsewhere in this subpart prior to May 17, 2017: (a) On or about October 30, 2019, employees who were replacing the old guard rail system on the roof of the train shed building had not been trained by the employer on the use and limitation of the fall protection system in use.

29 CFR 1910.29(b)(3): Guardrail systems were not capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied in a downward or outward direction within 2 inches (5 cm) of the top edge, at any point along the top rail. (a) On or about October 30, 2019, employees were exposed to a fall hazard while using stairs to access the first floor of the main building. The West side handrail of the steps leading to the 1st floor of the main building, had broken welds.
