Nation Pizza Products LLC was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1910.147(c)(7)(i)(A) which was contained in OSHA Inspection Number 1095779, Citation number 1, Item number 3b and was affirmed as a final order on December 16, 2015 with respect to the workplace located at 601 E. Algonquin Schaumburg, Illinois, 60173.    In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase, or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):(a)     (a) Topping Room 4 - On or about October 27, 2019, employees were exposed to machine hazards associated moving and rotating parts during sanitation activities including dismantling of equipment and dry cleaning of equipment such as but not limited to the Urshell Shredder 1 machine. The employer failed to implement energy control application steps as the machine was not shut down and unplugged and the pneumatic air was not dissipated to perform the sanitation work [per the 1910.147(d)(2) requirements]. As a result , the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(I)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.    An employee suffered an amputation to the middle finger on the right hand when performing dry cleaning/sanitation work on the Urshell Shredder 1 machine while the equipment was plugged in and running.      Nation Pizza Products LLC was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1910.147(d) which was contained in OSHA Inspection Number 1387217, Citation number 1, Item number 1 and was affirmed as a final order on July 22, 2019 with respect to the workplace located at 601 E. Algonquin Schaumburg, Illinois, 60173.     Nation Pizza Products LLC was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1910.147(d) which was contained in OSHA Inspection Number 1095779, Citation number 1, Item number 3a and was affirmed as a final order on December 16, 2015 with respect to the workplace located at 601 E.
