(a) At the residential construction site - the employer did not ensure that a Truck Mounted Concrete Pump, Model KYM 34XT Truck# 1 M2K189C6 YM0140, Vehicle# 3404, was identified as unsafe, rendered inoperable and/or taken out of service due to a defective safety interlock device on the hopper gate, thereby exposing employees to amputation hazards when cleaning the concrete hopper, on or about October 22, 2019.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) On or about October 22, 2019 at 4209 W Wheatfield Lane, Dunnellon, FL an employee suffered a work-related injury and was hospitalized. The employer was aware of the in-patient hospitalization on October 23, 2019 at or around 10:30 A.M. The employer notified OSHA on October 25, 2019 1:43 P.M. of the work related hospitalization.
