29 CFR 1926.453(b)(2)(v): A personal fall arrest system (Harness) shall be worn and a lanyard attached to the boom or basket when working from an aerial lift:   a) On or about October 24, 2019, at the above addressed jobsite, an employee working in a telescopic boom aerial lift was not properly attached to the boom or basket by the use of a lanyard, thereby exposing the employee to the hazards associated with a fall of more than fifteen (15) feet.
