An employee was injured when exposed to nip-point hazards when using a rubber glove held in the left hand to scrape loose paper from the adhesive spreader rollers with the equipment operating.    b. On or about November 1, 2019, the employer did not ensure the energy control procedure established for the Black Brothers adhesive spreader clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance. An operator was exposed to nip-point hazards when cleaning the adhesive spreader rollers with a long handle brush with the equipment operating.
