On or about November 4, 2019, employees were exposed to a fall of approximately 22 feet while performing roofing activities on a residential home located at 5509 Timberlane Trail, Hudson, Ohio 44236. The employees were working on all areas of the roof and the employer did not ensure that they were using fall protection.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed. On or about November 4, 2019, employees were exposed to a fall of approximately 10 feet while performing roofing activities on a residential home located at 5509 Timberlane Trail, Hudson, Ohio 44236. The employees were using an A Frame ladder to access the upper level of the home and the employer did not ensure that they were using the ladder for the purpose it was designed.
