(a)  Simplot, Don Plant, Boiler #1:  On or about October 25, 2019, the controlling em0ployer, J.R. Simplot Company, while conducting steam blowouts on the super heaters, inadvertently heated up the soda ash wash and it condensed out into the boiler and exposed sub-contracted Guy Nielson Company employees to respiratory illness.
