29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1 Item 1 for employees exposed to overhead hazards and no protective helmets provided, Citation 1 Item 2, for employees being exposed to fall hazards due to working levels of scaffolds not being fully planked, Citation 1 Item 3 for employees being exposed to fall hazards due to the lack of safe access to the scaffold, Citation 1 Item 4 for employees being exposed to fall hazards during masonry activities due to lack of fall protection, Citation 1 Item 5 for employees being exposed to struck by hazards due to the lack of a toe board on the scaffold, and Citation 2 Item 1 for employees being exposed to fall hazards while working from a roof due to the lack of fall protection.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about November 4, 2019, employees were exposed to a struck by hazard while performing masonry activities on a residential home located at 6402 Forrest Edge Drive, Hudson, Ohio 44236.  An employee was working from the upper levels of a supported scaffold with tools and materials that could have fallen to the lower levels and the employer did not ensure that the lower employees were wearing protective safety helmets.

Crossbraces were used as a means of access:  On or about November 4, 2019, employees were exposed to a fall of approximately 14 feet while performing masonry activities on a residential home located at 6402 Forrest Edge Drive, Hudson, Ohio 44236. On or about November 4, 2019, employees were exposed to a fall of approximately 10 feet while performing masonry activities on a residential home located at 6402 Forrest Edge Drive, Hudson, Ohio 44236.  An employee was working on the roof of the home and the employer did not ensure that he was using fall protection.   Classic Stoneworks was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1392333, citation number 1, item number 1a and was affirmed as a final order on 5/13/19, with respect to a workplace located at 6401 Miller, Brecksville, Ohio 44141.
