29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: a) Production Area, the strain relief collar of the male plug to the Hobart vertical floor mixer had loosened and allowed the outer sheath to pull out, thus placing tension on the conductors when the mixer was unplugged, on or about, November 4, 2019. ABATEMENT CERTIFICATION REQUIRED
