(a) East and west side of newly constructed Chase Bank - On or about November 2, 2019, the employer did not instruct employees on how to recognize and control hazards associated with working in an area where uncovered holes where present and on the safety rules and precautions associated with the operation of a JLG Ariel lift , thereby exposing employees to a trip and fall hazards.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated. (a) West side of the newly constructed Chase Bank facing Abercorn Street - On or about November 2, 2019, the employer did not ensure that an employee operating a JLG aerial lift to install tower trusses on the roof of the Chase Bank wore a harness with lanyard attached to the boom or the basket.

(a) ATM Area - On or about November 2, 2019, two employees installing tower trusses for the ATM building were exposed to tripping or falling into four (4) uncovered holes that were 6 feet in width, 6 feet in length and 1 foot 8 inches in depth.
