29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a.) 160 Hillside Ave., Nutley, NJ  Employees did not utilize any means of fall protection while engaged in construction activities from the roof of a residential home, which exposed them to an approximate 12ft. fall hazard, on or about 11/04/2019.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:   a.) 160 Hillside Ave., Nutley, NJ  Employees were engaged in construction activities from a ladder jack scaffold without utilizing any means of a personal fall arrest system, exposing them to a fall hazard of approximately 12ft. to the next lowest level, on or about 11/04/2019.
