OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish to each employee employment and a place of employment which was free from recognized hazards, which were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to struck-by hazards while operating a crane without pendant control station function labels or without a functioning safety latch on the hook. This violation occurred on or about November 1, 2019, and times prior thereto: a. In building C, employees were exposed to struck-by hazards when the pendant control station, operating the QC1 15-ton crane, did not have function labels. b. In building C, employees were exposed to struck-by hazards when the pendant control station, operating the QC2 15-ton crane, did not have function labels. c. In building C, employees were exposed to struck-by hazards when the pendant control station, operating the No.23 15-ton crane, did not have function labels. d. In building C, employees were exposed to struck-by hazards when the pendant control station, operating the QC4 15-ton crane, did not have function labels. e. In building C, employees were exposed to struck-by hazards when hoisting material using the QC1 15-ton crane without a functioning safety latch on the hook of hoist H1. f. In building C, employees were exposed to struck-by hazards when hoisting material using the QC1 15-ton crane without a functioning safety latch on the hook of hoist H2. g. In building C, employees were exposed to struck-by hazards when hoisting material using the QC2 15-ton crane without a functioning safety latch on the hook of hoist H2. h. In building C, employees were exposed to struck-by hazards when hoisting material using the QC4 15-ton crane without a functioning safety latch on the hook of hoist H1.
