29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) Jobsite, Buncher Industrial Park, Mol-Dok dock, Leetsdale, PA: On or before November 4, 2019 employees were not protected from falling approximately 20 feet onto a make shift work platform floating on the Ohio River when traversing from the upper area to the ladder. Employees had to climb through a railing and climb down approximately 4 feet of the dock wall onto a beam approximately 12 inches wide and then walk approximately 5-6 feet to a ladder that led down to the work area. There were no fall protection systems in place when climbing through the railing or along the beam to protect the employees from falling onto the platform or water below.

There was no grab bag for employees to use to assist in accessing the upper landing surface when traversing from the ladder.
