These hazards were created by the potential for a large Front End Loader to move or roll while not attended:  Work Site:  On, or about, November 1, 2019, the parking brake provided on the Komatsu Model WA270-1 Wheel Loader was not in operation.

29 CFR 1910.23(b)(10): Ladders having defects were not marked and taken out of service until repaired:  In the following locations, ladders were in service that were damaged:  A.  Line 1:  On, or about, November 4, 2019, a Werner extension ladder was used to gain access the roof of the interior packing room.  The ladder had a damaged side rail.  B.  Line 1:  On, or about, November 4, 2019, a Werner step ladder was used to gain access to the transition section of the inclined conveyor.  The ladder was damaged in that the bottom rung had broken off.    C.  Line 1:  On, or about, November 1, 2019, a Werner step ladder was used at the packing line to load salt into a hopper.  The spreader was broken and a back horizontal support member was bent.    D.  Soil Line:  On, or about, November 4, 2019, a Werner ladder was used to gain access to the block wall adjacent to the infeed hopper.  The ladder was damaged in that the side rail was bent near one of the rungs.

29 CFR 1910.23(c)(11): The employer did not sure that portable ladders used to gain access to an upper landing surface had side rails that extended at least 3 feet (0.9 m) above the upper landing surface:  On, or about, November 4, 2019, in the following locations, ladders that were used to gain access to a higher work surface did not extend at least three feet above the upper surface:  A.  Line 1:  A Werner ladder was used to gain access the roof of the interior packing room.  The ladder extended 23 inches above the roof level.    B.  Line 2:  A Werner ladder was used to gain access to the top of Hopper 2.  The ladder was 10 inches above the top of the hopper.  C.  Soil Line:  Employees used a step ladder to gain access to the frame of the Hamer-Fischbein packing machine to change the gate adjustment for variations in production runs.  The top of the ladder extended about two inches above the frame of the machine where the employee would stand.
