On or about November 5, 2019, employees were exposed to slip, trip and fall hazards when the stairway leading to the water heater and water tank area in the boiler had a tread depth of only three inches.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly.  Employees working in the Marinette location of Verrette Materials, Inc. were using a fabricated lifting attachment for moving Texas Weights weighing 3,500 pounds. The attachment did not have the manufacturers prior written approval and no instructions were attached to the lift indicating capacity, operation and maintenance changes.

29 CFR 1910.215(b)(9): Exposure adjustment. Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, shall be constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel. The maximum angular exposure above the horizontal plane of the wheel spindle as specified in paragraphs (b)(3) and (4) of this section shall never be exceeded, and the distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top shall never exceed one-fourth inch. (See Figures O-18, O-19, O-20, O-21, O-22, and O-23.)  Employees were exposed to potential struck-by hazards when the tongue guard of the Delta 8" Bench Grinder was not attached to the grinder.

29 CFR 1910.252(b)(2)(iii): Protection from arc welding rays. Where the work permits, the welder should be enclosed in an individual booth painted with a finish of low reflectivity such as zinc oxide (an important factor for absorbing ultraviolet radiations) and lamp black, or shall be enclosed with noncombustible screens similarly painted. Booths and screens shall permit circulation of air at floor level. Workers or other persons adjacent to the welding areas shall be protected from the rays by noncombustible or flameproof screens or shields or shall be required to wear appropriate goggles.  Employees working in areas adjacent to arc welding work were not protected from eye contact with hazardous levels of ultraviolet radiation by welding screens or goggles.

29 CFR 1910.253(e)(5)(v): Hose showing leaks, burns, worn places, or other defects rendering it unfit for service shall be repaired or replaced.  Employees using the fuel gas torching system in the back maintenance area were exposed to burn hazards when the fuel hose connected to the torch had a tear covered with electrical tape and rip with exposed fiber core protrusions at the coupling.
