29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about 11/05/2019, an employee from SNL Painting Inc. was working off of the roof of a residential structure, located at 1031 Whitmoor Drive, St. Peters, MO.  He was not utilizing guardrail systems, a safety net system, or a personal fall arrest system.  The roof pitch was approximately 10:12 with a fall height of approximately 12 feet 8 inches.

29 CFR 1904.29(a): The employer did not use an OSHA 300, 301 and 300A Form or equivalent:   On or about 11/05/2019, during a residential Fall Hazard inspection at 1031 Whitmoor Drive, St. Peters, MO, the company owner, Nick Lograsso, stated that the company has not kept OSHA 300 forms.  The owner stated that he did not know what an OSHA 300 form pertained to.  SNL Painting employees approximately 20 employees.
