29 CFR 1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:    West side of the residential construction site: On or about November 05, 2019, and at times prior thereto, employees were exposed to a trip and fall hazard while, using a wooden ladder for roof access and egress, surrounded by debris, and wood plank on the floor.
