29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: 8603 TX-151 parking lot: On or about November 04, 2019 and at times prior thereto, employees were exposed to fall hazards while working 15 feet above ground in an aerial lift, and wearing fall harnesses with the attachment point located below the shoulder blades.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used. Parking lot of 8603 TX-151 Suite 101: On or about November 04, 2019 and at times prior thereto, employees wearing fall harnesses with an unsecured leg strap while working in an aerial lift 15 feet above ground, were not trained in correct use and operation of wearing fall harnesses and were exposed to fall hazards.
