29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a. On or about November 5, 2019, at an overhead doorway at the back of the building, the employer did not ensure that an employee, working from an extensible boom lift platform, was in use of a lanyard attached to the boom or basket and was not restrained from being thrown or falling from the extensible boom lift platform. This exposed the employee to a fall hazard.
