This condition exposed employees to falling object hazards.

29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems: C&L Water Solutions, Inc. at 104th and Melody Ave. Northglenn, CO 80234: On or before 11/6/19 the employer did not ensure that inspections of the excavation and adjacent area were inspected in accordance with this standard. Corrective actions were not made prior to employees entering the 9 foot deep 14 foot long and nine foot wide trench with class C soil. The trench lacked protective shielding on two ends. This condition exposed employees to a fall hazard of nine feet and trench collapse hazards.

29 CFR 1926.651(i)(3): Sidewalks, pavements, or appurtenant structures had been undermined and a support system or another method was not provided to protect employees from the possible collapse of such structures: C&L Water Solutions, Inc. at 104th and Melody Ave. Northglenn, CO 80234: On or before 11/6/19 the employer did not protect employees from injury in that unsupported pavement which had been undercut was used as a walking working surface next to the nine foot deep trench. This condition exposed employees to a fall hazard of nine feet and struck by hazards.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): C&L Water Solutions, Inc. at 104th and Melody Ave. Northglenn, CO 80234: On or before 11/6/19 the employer did not protect employees from injury in that employees working inside a 9 foot deep 14 foot long and nine foot wide trench with class C soil lacked protective shielding on two ends. The trench box being used only provided protection on the long axis of the trench. This condition exposed employees to trench collapse hazards.
