29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: Jobsite: The employer allowed employees to use extension cords that were connected to several relocatable power taps in a manner that were not consistent with the labeling of the electrical equipment that was used to supply power to a Mikita circular saw. On or about September 30, 2019, an electrical fire occurred at the jobsite while an employee was using the power saw due to improper use of electrical equipment.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: Jobsite: The attachment plug of an extension cord that was plugged into a relocatable power tap to supply power to the Milita circular saw was missing the grounding pin. On or about September 30, 2019, an electrical fire occurred at the jobsite while an employee was using the power saw due to improper use of electrical equipment.
