29 CFR 1910.22(d)(1): (1) Walking-working surfaces are inspected, regularly and as necessary, and maintained in a safe condition; (a) Wash tank - On or about November 1, 2019 the employees working on the wash tank did not follow their own procedure to review all tasks to assure all safety measures were in place, allowing an employee to step on the tank and the lid move and the employee fell into the acid.
